Patient-Centered Care Teams

Primary care is more than day-to-day medical care. Primary care gives access to all the medical and mental health services you need, when you need it, from dedicated, caring, highly-skilled providers and staff.

Our Patient-Centered, Care Team approach offers coordinated health services for you and your family. You will have access to your own primary care provider (physician, physician assistant, or nurse practitioner) plus a Care Team of behavioral health providers, nurses, assistants, and specialists dedicated to your health and wellness. The Care Team will be there for you when you are sick, will help you stay healthy, will keep you connected, and will help you reach your health care goals.

 

We maintain four guiding principles in providing care:

Access for All

SCCH welcomes all people of all ages, regardless of economic status, disability, sexual orientation or gender identity, immigration status, ability to pay, or any other circumstance. Our services are available in person, by phone, or by video, any day and any time.

Quality

We deliver high-quality care through compassionate, dedicated, and skilled medical and behavioral health providers, nurses and medical assistants. Integrated behavioral health, addiction treatment, case management, health education, acupuncture, and chiropractic services ensure a whole person orientation to care. Our electronic health records system shares important health information with you, keeps you connected to your Care Team, and ensures you receive the most current health services including vaccinations and preventive care.

Comprehensive Services

We provide comprehensive services including primary care for all ages, women’s health & contraception, pediatrics, mental health and substance use treatment, chronic disease management, prenatal care, health education, acupuncture, chiropractic services, and more. Our enrollment specialists help you apply for health insurance, CalFresh, and other programs, and resources available to our patients.

Patient-centered Care

You, your family, your health and wellness, and your experience are at the center of everything we do. Our approach focuses on the patient through personalized care, advocacy and health education delivered with cultural humility and respect. Quarterly patient experience surveys tell us that our patients feel respected, heard, and well-served.

 

Most insurances accepted, including Medi-Cal, MediCare, Healthy Kids, Anthem Blue Cross, Blue Shield, and Health Net.  Sliding fee for those without insurance. All patients will be served regardless of ability to pay.
